The Path Forward

  • All initial consultations take place in person at the private practice in my home in Bennington, Nebraska. This face-to-face meeting allows us to establish a personal connection and ensures I can provide the most comprehensive assessment of your needs.

  • To complete your consultation request, I'll reach out via email to securely collect your payment information. I'm committed to providing dedicated time and attention to each patient, which is why appointments require 24 hours' notice for cancellation or rescheduling. Appointments cancelled with less notice will be subject to a $100 full appointment fee, not covered by insurance. This policy helps ensure I can provide the highest quality care to all my patients.

  • After booking, you'll receive access to your intake forms, which must be completed before your appointment. This helps us make the most of our time together.

  • During this comprehensive visit, we'll discuss your complete health history, explore your weight management goals, and begin developing your personalized treatment plan. This is your time to share your story and ask questions in a judgment-free space.

  • I'll order the necessary lab work at an outside facility and ask you to research which weight management medications your insurance may cover. These steps help us make informed decisions about your treatment options.

  • Once your lab results are available (typically within one week), we'll meet again to review your results, discuss medication options based on your coverage, and finalize your personalized treatment plan.

  • If medication is part of your plan, we'll schedule monthly 30-minute follow-up appointments to monitor your progress, address any concerns, and make adjustments as needed.

Important Information

Location & Eligibility

I serve patients local to Bennington (where my practice is located), Omaha, Elkhorn, and surrounding areas. Due to licensing regulations, I can only provide care to patients physically located in Nebraska. All initial consultations must be conducted in person, though some follow-up appointments may be available via telehealth for established patients.

Insurance Accepted

I am in-network with most Blue Cross Blue Shield, United Healthcare, Cigna, UMR, and Tricare. As always, confirm with your insurance that Lupo Preventative Medicine is in-network for your plan. If you are out-of-network, I offer cash pay rates.

Cancellation Policy 

If you are not able to keep an appointment, please provide a 24-hour cancellation notice. If this does not occur, a no-show/late cancel fee of $100.00 will be billed. If there are three no-shows within one year, you will be asked to transfer to a different practice.

If you are running more than 15 minutes late for your appointment, I’ll do my best to accommodate you. However, on certain days it may be necessary to reschedule your appointment.
